Head of CIS Reablement and OT Services

Working on behalf of a Local Authority, Aatom Recruitment has a new opportunity for a Head of CIS Reablement and OT Services on a 3 month contract with the possibility for further extension.

Main responsibilities:

Demonstrate your passion for promoting people’s individuality, diversity and rights to:

• Provide an effective and efficient service to vulnerable adults with identified rehabilitation and care needs by working with individuals and their Carers, within their Community. Supporting a service to enhance and promote independent living in local communities with an enabling personal centred approach.
• You will be committed to timely and safe discharges by managing a responsive, safe , caring, effective and an efficient service.
• Operational oversight of the external Complex OT service provision in WCC in conjunction with Contracts and commission colleagues,
• Make sure that services offered are responsive to the needs of all users, including their religious, cultural and linguistic needs, and the needs of users with disabilities.
• Manage CIS budget effectively and efficiently ensuring that the client gets value for money at all times balanced with excellent service delivery
• Directly manage and support the CIS Reablement Team and OT managers across our client’s area and RBKC and support to the CIS reablement Multi-disciplinary teams.

Use your practical ability to apply and effectively use theoretical frameworks, Social Care Policies, Rpyal College of OT guidelines and research evidence you will:

• Undertake care co-ordinator responsibilities within the Care Programme Approach and within a multi-disciplinary framework.
• Oversee reviews and attend multi-disciplinary meetings, to identify and recommend the most appropriate packages of care available.
• Ensure that appropriate plans for hospital discharge to the community are made and followed through.
• Identify any potential needs/vulnerabilities of Service Users children and work in partnership with Children’s Services as appropriate.
• Identify the most appropriate carers for Service Users and offer statutory Carers Assessments and Care Plans.
• Support pathway and process development/implementation and review to provide a personalised streamlined safe and effective CIS service delivery
• Representing the CIS service and ASC at strategic multi agency meetings and forums
• Keeping abreast with latest trends and developments in intermediate care and preventative services and apply to service with a view to continuous service improvement
• Work collaboratively with internal and external stakeholders, demonstrating the expected leadership values to:
• Be actively involved in the provision of services to vulnerable adults with complex support needs and their Carers.
• Fully participate in the assessment or care management service.
• Promote the development of an effective and efficient service ensure an integrated response to service user needs.
• Determine an accurate and sensitive assessment of service user needs.
• Identify appropriate resources within the area, the Health Trust, the local community, non-statutory organisations and primary care to ensure the Service User receives the most appropriate packages of care available.
• Represent the service at collaborative, integrated Health and ASC platforms
• Establish and enhance key partner relastionships with CCGs, Acute hospital trusts, third sector organisations to ensure effective and timely service delivery

Requirements:

You will have demonstrable knowledge experience and understanding of:

• Significant management experience in a similar Health or Social Care setting
• Extensive experience of working within ASC and/ or Health Care Services
• Experience of working within integrated multi disciplinary services, carers, families and other professionals
• Experience of policy development, strategic planning, development of governance systems and change management programmes. Successful multiagency & partnership working
• Robust understanding of ‘whole system ‘ working
• The, Care Act 2014, the Care Programme Approach, the NHS LT Plan, and understanding of Health & Social Care integration and direction of travel
• An excellent command of written and spoken English which is appropriate for the effective performance of the role
• Excellent presentation skills
• Ability to use Word. Windows 10, outlook, Teams and Excel, IT systems and patient record systems
• Skilled decision maker with sensitivity to political and organisational issues.
